Effective March 1, 2023:

1. Awards for wig funding must be used within 90 days of notification, unless an extension is submitted and approved in writing by Wigged Out. Please submit all extension requests to hello@wiggedout.org. After 90 days, the award will be forfeited and applicants must reapply.

2. Wigged Out funding is restricted to residents of Arizona. All applicants must have a valid Arizona address in order to be considered for funding. Exceptions may be made for non-residents currently receiving treatment at an Arizona medical institution, with proof of treatment in the form of a doctor’s note or other documentation. Requests for exceptions must be submitted in writing to hello@wiggedout.org.

3. In order to be eligible for consideration for funding from Wigged Out, all applications must include proof of medical diagnosis which has resulted in hair loss, a prescription for a wig, or doctor’s note confirming medical reason for hair loss.

4. Wigged Out is unable to offer reimbursements directly to applicants. Wigged Out will provide payment in the form of a credit card made available for partner boutiques to use to charge the approved amount directly to Wigged Out, upon notification of approval from Wigged Out staff. Wig boutiques may, at their own discretion, offer reimbursements in the original form of payment to their customers who apply to Wigged Out and are approved for funding. Submission of an application does not guarantee approval for funding or award amount.

5. Receipts for wigs purchased with funding from Wigged Out must be submitted to Wigged Out staff via mail or email within 30 days of the purchase transaction.

6. Wigged Out staff will notify applicants and wig boutiques of approval status and award amount within 7 - 10 business days of submission date.

7. The Wigged Out credit card may not be used for applicant purchases until approval and award amount has been received in writing from Wigged Out staff. Successful submission of an application does not guarantee approval or award amount.

8. Individuals are eligible to receive one (1) Wigged Out award per year. Applicants may reapply one calendar year from the date of their approval to be considered for an additional award.
